I have several special coins for you to look at. The first is this 10 cents coin:
Incused welt across the reverse of a 1967 10 cents coin.  
What could have caused it?

Answer: 
This is a Lamination Errors.
Appears as flakes or peels on a coin's surface.

This is caused by an improper alloy mix or bubbles and/or foreign materials trapped in the metal of the Planchet.
